Our community outreach support team support people with a learning disability or autism in their own homes, in the community, at our local bases and through shared social and sporting activities and events. We can offer support to people who are self-funding, receive direct payments or managed care packages through social services and support people across most North, East London boroughs.
We work with people to identify what they want from their support and to match a support worker best suited to each individual. Support is provided at the times and places the person chooses and within a co-produced, outcomes based support plan.
Our outreach team have a wealth or experience and success stories helping people gain and maintain independence and redoing social isolation.
There are a wide number of things our community outreach support team can help with such as:
- managing money and paying bills
- filling out forms
- support to manage health conditions and attend appointments
- socialising and meeting new people
- managing a tenancy
- getting employment or volunteering opportunities
