Outreach Support

A day out at Kew Gardens

A day out at Kew Gardens

Outward has provided services to people with learning disabilities for over twenty years, as well as to people with mental health needs. Our Outreach Support teams in Waltham Forest and Tower Hamlets build upon this experience to provide managed support to people with these disabilities in a variety of settings. The teams consist of permanent contracted staff with backgrounds in Social Services, Health and the voluntary sector. The majority of staff time provided will always be by permanent staff to ensure consistency and uphold the quality of the service. The Outreach Support service aims to provide for individual needs and wishes respecting their disability, race, religion, colour, culture, gender, sexuality and age.

The Outreach Support service aims to promote individuals choice and rights across their local area and beyond in order to enable them to become active and valued members of the community. Every service users service agreement is reviewed each year to take into account any change in need ensuring that the support plan meets the service users needs and aspirations and is person centred.

Where the Outreach Support team is providing managed support services to an individual, the team will detail services provided in a Service Agreement which will be negotiated with the individual (and their Care Manager or other appropriate professional where necessary).

Services will above all respect people with learning disabilities or mental health issues as "people first", with their individual needs, wishes and aspirations. In the provision of services and in work with other agencies, the Outreach team will strive to develop and maintain services that accord people dignity and respect.
